Wounded in the forehead by shrapnel 1/7/1944 at 70 Brigade HQ, map reference 871681.

Evacuated to 147 Advanced Dressing Station, then to No 3 Casualty Clearing Station and then on to No 79 General Hospital.

Father's address as Next of Kin is given as 4, Inverton Road, Nunhead, London SE 15.
